Description
Root Cause Analysis is often reduced to a Fishbone diagram and a few 5 Whys. In real manufacturing investigations, that is rarely enough.
This practical course focuses specifically on D4 - Root Cause Analysis and uses one realistic manufacturing case throughout: an ADC12 aluminium high-pressure die-cast housing that fails a customer pressure-decay leak test.
Rather than teaching isolated textbook examples, we progressively investigate the same problem using different RCA techniques. You will see why each tool is introduced, what question it is intended to answer, what it can and cannot prove, and how its output feeds the next stage of the investigation.
You will begin with Is / Is Not Analysis to establish the problem boundaries and identify useful distinctions. You will then use stratification and Pareto analysis to identify meaningful patterns without confusing concentration with causation.
Next, Fishbone / Ishikawa analysis is used to generate plausible hypotheses across People, Machine, Method, Material, Measurement and Environment. Cause & Effect analysis then forces those hypotheses to be tested against the effects and evidence that should exist if they were true.
Fault Tree Analysis takes the investigation further by structuring the failure using AND / OR logic and separating occurrence and escape pathways. Physical failure evidence, material checks, dimensional evidence, process trends, vacuum-performance data and maintenance evidence are then used to eliminate weak explanations and strengthen the remaining causal pathways.
The course culminates in a Causal Verification Matrix, Hierarchical 5 Why and conventional 5 Why analysis. These tools distinguish occurrence, escape/detection and systemic causes and produce a final evidence-based D4 conclusion.
The ADC12 case is deliberately progressive. Learners are encouraged to pause, form hypotheses and decide what evidence they would seek before the next evidence set is revealed. The aim is to develop an investigation mindset, not simply template-filling ability.
By the end, learners should understand both how the individual RCA tools work and how they fit together into a disciplined D4 investigation.
